No Call, No Show Appointments Policy

If you cannot make the appointment for whatever reason, please kindly call our office at least 24 hours prior to your first visit with the dentist so that we can either reschedule you or use the time slot for other patients who need our time/services. Please understand that in case of repeated no-calls and no-shows we will not be able to give scheduled appointments to the patient anymore and will instead be forced to place the patient on a walk-in status where the patient may experience significant wait time. In extreme cases we may also resort to inactivating patients from our care as well. Thank you for your understanding and cooperation.

* If the patient has no periodontal disease, a normal cleaning will be done; however, if a deep cleaning is necessary, it will require its own treatment plan and fees and a normal cleaning will not be done in its place.

Consent Forms

Below please find the consent forms our office uses for the various dental procedures we provide. Patient consent is necessary for all dental work and we want our patients to fully understand the possible risks/dangers of each procedure before participation. If you have any questions, feel free to ask Dr. Tan prior to consent. Thank you.

I found this dental office about a year ago I transferred from another Dental office. DR.Tan is wonderful he really cares about his patients and their Dental health. The office is clean well organized and the staff are very welcoming and have excellent customer service, even the bathrooms are clean. Dr Tan takes his time with the patients and answers all of your questions or concerned. The patient offices are clean organized I love that they sanitize everything all of the instruments all of the equipment the telephone remotes Etc I really appreciate the cleanliness of the office and the extra precaution to the patience towards germs. I am so glad I found this office I will continue to go here and recommend others if you're looking for a great dental office this is the one!
